If you hear "es könnte passieren, dass," you should pay attention. Very often, it signals concern about an unfortunate event, similar to "it might happen that," "... could potentially," or "there is a risk of."

In my experience, the German phrase "es könnte passieren, dass..." usually indicates that something worse could happen. Sometimes, a clear threat is made. But just the conjunctive could be used as well: "Er könnte wütend werden". 
"He might get angry."
”彼は怒り兼ねない。”
A clear threat could come in the form of:
"Wenn es nicht besser wird, könnte es sein, dass ich meine Kündigung demnächst einreichen werden."
